Collection Instructions: 

1. Submit cord blood.

2. Mother and baby demographics must be indicated on tube. Label as follows:

Mother’s Label

1. Mother’s name

2. Mother’s hospital ID number

3. Time and date of draw

4. Initials of collector

Baby’s Label

1. Name of  baby (last name, boy or girl)

2. Baby’s 6 digit hospital ID number

3. Birthdate

4. Initials of labeler

5. Pink cord blood sticker
